Dear Parents and Guardians,

Halton Public Health is providing an immunization clinic for Middle One (grade 7) students at Appleby College on Thursday, October 10, 2024. The clinic will run from 9:00 a.m. to 2:00 p.m. and faculty will bring groups of students to the clinic throughout the day. Please click here to review a parent information letter. 

Public health nurses will offer the following three vaccines (free) to eligible Middle One (grade 7) students who have parental consent by submitting a School Vaccination Consent Form: 

• Meningococcal Conjugate-ACYW-135, required for school attendance (1 dose)
• Human Papillomavirus – HPV, recommended (2 doses given 6 months apart)
• Hepatitis B – HB, recommended (2 doses given 6 months apart)

Please click here to access the School Vaccination Consent Form. The completed form must be e-mailed to Amy Cronin, Health Centre coordinator/nurse at acronin@appleby.on.ca, or dropped off to the Health Centre by Friday, September 20, 2024 for your child to be eligible to receive their vaccines. Only students with a signed consent form will be immunized on the day of the clinic.
