Dear Parents and Guardians,
Halton Public Health is providing an immunization clinic for Middle School students at Appleby College on Monday, May 3, 2027 and faculty will bring groups of students to the clinic throughout the day. Public health nurses will offer the following three vaccines (free) to eligible Middle One (grade 7) students and additional doses will be provided to Middle Two (grade 8) students who have not yet completed their vaccine series who have parental consent by submitting a School Vaccination Consent Form:
- Meningococcal Conjugate-ACYW-135, required for school attendance (single-dose vaccine)
- Human Papillomavirus – HPV, recommended (2 doses given 6 months apart)
- Hepatitis B – HB, recommended (2 doses given 6 months apart)
Please note that Middle Two (grade 8) students who have not yet received their Meningococcal Conjugate-ACYW-135 immunization are encouraged to do so before the school-run clinic on Monday, May 3, 2027. Because school vaccination requirements are expected to be enforced before the clinic date, students who are missing their Meningococcal Conjugate vaccine will need to have this immunization completed in advance to avoid any interruption to their attendance.

The completed form must be e-mailed to healthcentre@appleby.on.ca, or dropped off to the Health Centre by Monday, September 21, 2026 for your child to be eligible to receive their vaccines. Only students with a signed consent form will be immunized on the day of the clinic.
