To avoid school suspension, Public Health requires parents/guardians who have not updated their child’s records to complete the following actions as soon as possible:

- If the student requires immunization, arrange for them to receive the vaccine doses through their regular health care provider, or at a Public Health Community Immunization Clinic. Appointments will be available leading up to the Thursday, March 21, 2024 suspension date. Visit halton.ca/immunize to check clinic availability and book an appointment.
- Parents/guardians must promptly report all vaccines administered by health care providers to Public Health by visiting halton.ca/immunize.

Please review the parent information letter which provides additional details about the study. If you have any questions about COMPASS, please contact Mary Brajkovic, director of wellness. There are no known or anticipated risks to participation in this survey.
